Ask: “If we can only ship with 50% of planned features, who decides which half?”
If the answer is “we’d need to discuss that as a team,” you have a governance problem masquerading as a collaboration process.
What Actually Works:
One person owns success. That person has unilateral authority to cut features, change priorities, and override stakeholder preferences. Everyone else has input; one person has decision rights.
If you’re not willing to grant that authority to someone, you’re not ready to build custom software. You’re funding a negotiation process that happens to involve developers.
2. Requirements Defined by Features, Not Outcomes
The Setup:
Requirements gathering produces a feature list. “The system must have user authentication, email notifications, a dashboard with six widgets, export to PDF, and integration with Salesforce.”
The feature list is detailed. The why is assumed. No one writes down: “This system must reduce invoice processing time from 4 hours to 20 minutes, or it fails.”
Why This Kills Projects:
Features are guesses about solutions. Outcomes are definitions of success. When requirements are feature-driven, the project optimizes for shipping functionality rather than solving problems.
This creates two failure modes:
- You build the wrong thing correctly. All features ship, but the system doesn’t solve the problem because the features were wrong guesses.
- You negotiate away the right thing. Budget pressures force feature cuts. Because no one defined success in outcome terms, critical features get cut alongside nice-to-haves.

Review your requirements document. Find the success definition. If it says “system will include features X, Y, Z,” you have a feature list, not requirements.
If it says “system will reduce processing time by 80% while maintaining 99.9% accuracy,” you have requirements.

Define success in measurable outcomes first. Then map features to outcomes. Every feature should answer: “Which outcome does this enable, and how do we know?”

3. Budget and Timeline Derived from Hope, Not Data

Vendor B says: “12 weeks, $120K.”

You pick Vendor A. Not because their approach is better, but because 8 weeks fits your launch goal and $80K fits your budget.
No one asks: “What’s included in 8 weeks that’s excluded from 16 weeks?” No one validates: “Has this vendor delivered similar projects in 8 weeks before?”
Why This Kills Projects:
Software projects don’t fail because developers write code slowly. They fail because the agreed timeline was fantasy from the start.
The pattern:
- Week 4: “We’re a bit behind schedule, but we’ll catch up.”
- Week 8: “We need two more weeks for testing.”
- Week 12: “We’re launching with reduced scope.”
- Week 16: “We need to discuss additional budget.”
The optimistic timeline wasn’t a target that motivated hustle. It was a commitment that guaranteed failure.
The Tell:
Ask the vendor: “Show me three projects with similar scope. What was estimated timeline vs. actual delivery?”
If they can’t show you, or if every project delivered on time, you’re looking at either (A) a new vendor with no track record, or (B) a vendor telling you what you want to hear.
Neither is foundation for a realistic timeline.
What Actually Works:
Budget for the pessimistic estimate, not the optimistic one. If the gap between vendors is 8 weeks vs. 16 weeks, the real timeline is probably 20 weeks.
Add 50% to the most conservative estimate for unknowns. If that budget doesn’t work, the project isn’t viable yet. Pretending it is doesn’t change the math—it just moves the moment of truth from planning to crisis.
4. Integration Complexity Invisible Until Too Late
The Setup:
The new system needs to “integrate with existing tools.” The requirement sounds simple: “Pull data from Salesforce, push data to accounting system, sync with email platform.”
During scoping, integration is estimated at “2 weeks.” No one maps the actual data flow. No one audits API limitations. No one asks: “What happens when Salesforce is down?”
Why This Kills Projects:
Integration isn’t a feature—it’s a system of dependencies, each with failure modes, rate limits, authentication requirements, and data consistency challenges.
The 2-week integration estimate assumed:
- APIs are well-documented (they’re not)
- Data schemas align cleanly (they don’t)
- Rate limits won’t be hit (they will)
- Authentication is straightforward (it isn’t)
- Both systems stay stable during development (they won’t)
Reality: The “2-week integration” consumes 8 weeks, reveals data quality problems in existing systems, and surfaces political questions (“Who owns customer data in a conflict?”) that should have been resolved before kickoff.
The Tell:
List every system integration point. For each, ask:
- What API version are we using?
- What’s the rate limit?
- How do we handle authentication expiration?
- What’s the data refresh latency?
- Who gets paged when it breaks?
If those questions haven’t been answered, integration isn’t scoped—it’s a placeholder for “we’ll figure it out later.”
What Actually Works:
Audit integrations before development starts. Build proof-of-concept integrations for any system you haven’t connected to before. Assume integration will take 3x longer than estimated.
If that timeline breaks your launch plan, you have an integration problem that won’t get better by ignoring it during scoping.
5. No Agreement on What “Done” Looks Like
The Setup:
Development finishes. Vendor says: “Ready for launch.”
You test the system and find:
- Dashboard loads slowly with real data volumes
- Edge cases produce cryptic error messages
- Mobile experience is barely functional
- No user documentation exists
Vendor says: “Those weren’t in scope. Done means features are built.”
You say: “Done means production-ready.”
The argument about what “done” means happens after the budget is spent and the deadline has passed.
Why This Kills Projects:
“Done” is subjective until you define it objectively. Without explicit acceptance criteria, every stakeholder has a different definition:
- Developers: “Code matches the spec.”
- Project Manager: “Features are deployed to staging.”
- Product Owner: “Users can complete core workflows.”
- Executive: “System is generating ROI.”
These aren’t slightly different—they’re separated by weeks of work and thousands of dollars.
The Tell:
Ask: “What are the conditions required for us to accept delivery and make final payment?”
If the answer is vague (“system is working properly”) or feature-focused (“all planned features are built”), you have no shared definition of done.
What Actually Works:
Define acceptance criteria before development begins:
- Performance benchmarks (“Dashboard loads in <2 seconds with 10K records”)
- Error handling requirements (“Every user-facing error includes actionable next step”)
- Documentation deliverables (“Admin guide with screenshots for every workflow”)
- Browser/device support (“Works in Chrome, Safari, Firefox; responsive on mobile”)
These aren’t nice-to-haves. They’re the difference between “code is written” and “system is usable.”
If you wait until delivery to define “done,” you’ve already lost the negotiation.
The Hidden Pattern: Conflict Aversion
These five failure modes share a common root cause: the decisions that prevent failure are uncomfortable to make early.
It’s uncomfortable to tell stakeholders:
“You have input, but Sarah has final authority. If you disagree, Sarah decides.”

It’s uncomfortable to tell vendors:
“Show me proof you’ve delivered similar projects on time, or we’re not signing.”

“We will not negotiate on performance benchmarks. If it’s slow, it’s not done.”
So these conversations get deferred. Teams choose optimism over realism because realism forces conflict, and conflict before the project starts feels avoidable.
But deferring conflict doesn’t eliminate it. It moves it from the planning stage—where it’s cheap to resolve—to the development stage, where it’s expensive, or the delivery stage, where it’s catastrophic.

Red flag answer: “Depends on user feedback” or “We’ll know it when we see it”
Green flag answer: Specific metrics with thresholds (“Reduce processing time from 4hrs to 20min”)
3. What’s the Realistic Timeline?
Question: “Has this vendor delivered similar projects in the proposed timeline? Can they show proof?”
Red flag answer: “This is their estimate based on experience” (without specifics)
Green flag answer: “Yes, here are three comparable projects with actual delivery timelines”
4. Where Are the Integration Risks?
Question: “For every system integration, can we prove we can authenticate, pull data, and handle failure modes before development starts?”
Red flag answer: “We’ll figure out integration during development”
Green flag answer: “We’ve built and tested proof-of-concept integrations for every external system”
5. What Does “Done” Mean?
Question: “What specific, measurable criteria must be met for us to accept delivery and release final payment?”
Red flag answer: “All features built and tested”
Green flag answer: Written acceptance criteria including performance, error handling, documentation, and device support
If you can’t answer all five questions with green flags, you’re not ready to start development. You’re ready to do the work that makes development viable.
Why This Matters More Than Technical Skill
The irony: None of these failure modes are technical.
They’re not caused by:
- Choosing the wrong programming language
- Poor code quality
- Inadequate testing
- Slow developers
They’re caused by organizational decisions that happen before developers are involved.
You can hire the best development team in the world, and if you haven’t resolved ownership ambiguity, defined outcomes, validated timelines, scoped integrations, and established acceptance criteria, the project will fail anyway.
Technical excellence can’t compensate for structural failure.
What “Before the First Line of Code” Actually Means
The pre-development phase isn’t administrative overhead. It’s not “bureaucracy that slows us down.” It’s the work that determines whether the technical work succeeds.
Pre-development is where you:
- Resolve political conflicts before they become architecture decisions
- Surface hidden complexity before it becomes budget overruns
- Align stakeholders before their misalignment produces conflicting features
- Define success before “done” becomes a negotiation
- Validate timelines before optimism becomes crisis
Skipping this work doesn’t accelerate delivery. It just moves failure from the planning stage (where it’s fixable) to the delivery stage (where it’s not).
The Uncomfortable Truth
Most custom software failures are preventable and predictable.
The reason they happen anyway is that prevention requires making uncomfortable decisions:
- Telling stakeholders they don’t have equal authority
- Rejecting timelines that align with business goals but not technical reality

These decisions are uncomfortable. Deferring them is easy.

When to Build vs. When to Wait

Outcome 1: Ready to Build
All five questions have green flag answers. You have:
- Clear ownership with decision authority
- Measurable success criteria
- Validated, realistic timeline

- Written acceptance criteria
Action: Proceed with confidence. You’ve de-risked the project before committing resources.
Outcome 2: Fixable Gaps
Three or four green flags. Problems are known and addressable. You have:
- Identifiable blockers
- Path to resolution
- Stakeholder buy-in to fix gaps before starting
Action: Fix the gaps, then start development. Add 4-8 weeks to timeline for pre-development work. The delay is cheaper than the failure.
Outcome 3: Not Ready
Two or fewer green flags. You have:
- Political complexity without resolution path
- Unrealistic expectations that stakeholders won’t revise
- Integration dependencies you can’t validate
- No consensus on success definition
Action: Do not start development. Either:
- Option A: Invest in resolving structural blockers (might take months)
- Option B: Descope to something achievable with current readiness
- Option C: Cancel or defer until organizational conditions improve
Starting development when you’re not ready doesn’t demonstrate commitment. It demonstrates optimism over evidence.
The Counterfactual No One Talks About
Here’s the narrative no one wants to hear:
The projects that succeed aren’t always the ones with the best developers.
They’re the ones that resolved the five failure modes before development started.
The success story isn’t: “Our developers were amazing and saved the project.”
The success story is: “We did boring, uncomfortable pre-work, so developers never faced preventable crises.”
Nobody writes case studies about:
- The authority conflicts resolved in week 2 instead of week 16
- The timeline extended before contract signing instead of mid-development
- The integration audit that revealed a blocker before it burned two weeks
- The acceptance criteria argument that happened in scoping instead of at delivery
But those are the decisions that differentiate projects that ship from projects that die.
